Welcome to Reach Out
Personalising Day and Community Services
As we talked to colleagues across the country it became clear that many people working in Day Services were struggling to bring about real change.
There is a lack of clarity about the purpose of Day Services, real fear of change and no consistent way of capturing whether the services were leading to greater opportunity, choice and control for the people supported. It would be easy to just criticise but our response was to seek a number of partners to develop Reach Out.
Reach Out has been developed to support the process of developing and changing traditional day and community services, in order that they can engage with the community more dynamically, and support the people who use them to have real and fulfilling lives within that community.
Reach Out provides a process to help day and community services change in a way that reflects what people want. It suggests how to work towards developing better communities, and how to make sure that the changes work alongside the introduction of personal budgets.
Reach Out proposes eight Community Outcomes in relation to what day and community service change should be aiming for. The outcomes do not dictate any one type or style of support. They can be achieved in different ways for different people and what is most important is providing opportunity, choice and flexibility.
We believe that sharing, good practice and ideas, working together and focusing on supporting people to lead full lives is important. Reach Out is designed to encourage of all this and more!
